Coaching and healing sessions may augment therapy but do not replace it. In that spirit, by purchasing coaching and healing services from Jenna Dalton, you confirm that you have read and agreed to each statement in this document and wish to proceed: "I understand that the coaching and healing services I will be receiving from Jenna are not offered as a substitute for professional mental health care or medical care and are not intended to diagnose, treat or cure any mental health or medical conditions. I understand that Jenna is not acting as a medical professional when providing coaching or healing sessions. I understand and agree that I am fully responsible for my well-being during my coaching sessions, including my choices and decisions. I have the ability to give my informed consent, and hereby give such consent to Jenna to assist me in achieving such goals and aims. I understand that Jenna is not promising outcomes, and that the outcomes depend on me and my willingness to do the work. I understand that insurance does not cover coaching or healing sessions and that I am responsible for full payment before a session begins. I understand that Jenna will protect my information as confidential unless I state otherwise in writing. I understand that if I report abuse or neglect of a child, elder, or person with disabilities, threaten to harm myself or someone else, or if Jenna is ordered by a court to provide information or testify, that necessary actions will be taken and my confidentiality agreement is limited in this capacity. I understand that the use of technology is not always secure and I accept the risks of confidentiality in the use of Zoom, phone, email, text, and other technology." By reading this and proceeding with any purchase of services you hereby release, waive, acquit and forever discharge Jenna Dalton, including any agents, successors, personal representatives, executors, heirs from every claim, suit action, demand or right to compensation for damages I may claim to have.
